I don't trust them

I got a bill almost 2 months ago from them for labwork that I did not have done. I called 4 times to speak with someone and she said that I could not get the information about who billed me. She said she would mail me the details. I did not receive any such information and called again and again to have it sent. Then I received another statement from them. It was the same as the original. Once again I called but no answer. Two of the times that I called, their voice directory connected me but no one answered. I could hear someone on the other end shuffling and breathing a tiny bit so I yelled hello over and over and they finally answered as though they just got connected. They gave me the run around and then I got dinged on my credit a few days ago.
I'm appalled because I did NOT get a bill within even a month of that happening and I know all the medical work that I gotten done was sent to insurance and PAID. I didn't even see a doctor within a month of this alleged bill.
I called and said that I will pay it immediately IF they can tell me who billed me and for what. I have been saying that every time. I have never dodged payment, but I have a right to know who I am paying and for what. They are now saying that they cannot request that information from their client. I am not allowed to know who I owe money to?
I finally spoke to someone (a new "rep") who supposedly looked this up. Said it was from Stockton, but on my statement it says Oxnard, the next city over. I asked once more who they were collecting on behalf of. The guy said that he didn't know but that it said that I was uninsured. I am insured and all my doctors have that information. He said to call insurance and have them check. If they've paid it then that's my only proof to show that I did not get a bill. Otherwise, I have to pay it and they will not remove the mark on my credit.
I do not see how this legal in any way.
Also, some advice: If your debt is old, they may have purchased it and if you respond the debt can be collected. Otherwise (over 4-6 years) they can't collect anymore. http://www.huffingtonpost.com/creditcom/debt-collectors_b_1353676.html
